de_Dust 2 (Like Original CS:GO Map) x4
This is fully setup and (Shouldn't need any further setup)
to avoid any issues please use the whole server file and or copy the plugins folder to a 1.12.2 server (In short: run this on 1.12.2, it may be able to got below 1.12.2 but its not my fault for any errors!)
