Removed a Mod and he still has the Moderator pulldown on the top of his pages

Removed the user from all moderation usergroups primary and secondary.

Removed all custom user permissions (using "Set all to no") and he is still listed as having custom permissions.

FWIW I took over the forum probably five years ago. This is a 25 year old forum that's been imported from phpBB -> vBulletin -> vBulletin -> Xenforo.
